Liability Waiver

The Swing Dance Society at CSU is a registered student organization at Colorado State University.

We require that all students sign a release of
liability waiver once per academic year to participate in any lesson,
dancing, practice, competition, or performance.
We require all non-CSU students to sign a release of liability waiver each time
they participate in any lessons, dancing, practice, competition or performance.
This waiver indemnifies Colorado State University from any possible claims against them
brought about through participation in any of our events.

Your privacy is very important to us and we will never give your information
to anyone outside of the CSU offices that we report to.
